1. This registration form is for both morning and afternoon sessions of the seminar. The 12:10–13:00 midday session will have a separate registration form provided by the College of Medicine.

2. Except for the midday session, which will be held at Hall B, B2, International Research Building, all other sessions will take place at the IR630 Conference Room on the 6th floor.
